FIRST ANSWER QUESTION 1 ON THIS FORM, BEFORE MOVING ON TO FORM T4a FOR EACH OF THE CHILDREN BELOW 5 YEARS OLD IN THE HOUSEHOLD. FORM T4b SHOULD ONLY BE COMPLETED AT THE END OF THE INTERVIEW AND ONLY IF 1) IT IS A TASAF HOUSEHOLD AND IF 2) THE CHILD IS OLDER THAN 6 MONTHS OLD AND YOUNGER THAN 5 YEARS OLD.
