Interviewer instructions
For the answer option 'F. Immunizations', should be differenciate medical injection and immunization/vaccination, because at a glimpse they look the same. Medical injection is a treatment for an ill child, while vaccination is for directed to a relatively healthy child.

Description

Definition
Kind of health services:
A. Health check/consultation: checking of physically health/ or illness.
B. Obstetric check: include checking the fetus, fetal heart rate, etc.
C. Injection: drug delivery using a syringe. Not include BC injection.
D. Drugs/vitamin: drugs that recieved other than mentioned in prescription
E. Birth Control (BC): delivery, installation, or revocation of contraceptives, including treatment of complications due to the installation of an IUD, etc..
F. Immunizations: vaccination for children, pregnant women, bride and groom
G. Scalling: height and weight measurements
H. Traditional medication: non medically medication such as acupuncture, shinshe (traditional Chinese medication)
I. Dentist
J. Blood pressure
K. Supplementary feeding: usually given to the toddler (age 0 - 5 years) at integrated service post (Posyandu)
L. Birth/post natal care
M. Surgery: for illnesses those need surgery, specification minor surgery (patient locally anesthetized) that no need hospitalized
N. Rontgent/laboratory check: such as x-ray, blood check, urinary check, etc.
O. Circumcision
