Description
Definition
Month and year of birth only asked of ART aged 0-17 years. Ask month and year of birth of respondents in the Christian calendar and fill in the answer in the boxes. The first two boxes for month, and two boxes into two for 2-digit years are taken from the back. If respondents answered the month and year of birth instead of the Gregorian calendar, it must be converted. conversion can be found in appendix. If the respondent does not know his birth month then write in box 00 on the year of birth are to be filled.
