Interviewer instructions
Ask each household about the distance from the household to services such as schools, hospitals, place of work, etc. The distance should be stated in meters opposite each of these services as mentioned in the question. For example, write down 1 if the distance is 100 meters or less; 2 if the distance is more than 100 meters and less than 300 meters, and so on. There should be an answer to every question and the interviewer should help the household in making the estimation.
