A key part of the intervention was creation of personalized care plans by doctors, in consultation with their patients. Since they formed the key part of the intervention it was of paramount importance to assesses whether they contained some essential features and were tailored to patient's individual needs.\n\nThe following data pertains to the scoring of the care plans created by coordinators trained to administer the ECM program and introduce it to the participating doctors. The coordinators assessed 5 care plans, selected at random from each participating doctor. Each existing care plan was assessed across 6 dimensions, scored on 1-5 scale (as per the questionnaire), plus an open field for the coordinators to leave any comments. If a pre-selected care plan did not exists, the B0 survey question would reflect that. The evaluations happened during one of the coordinators' visits to the participating doctors and their teams, timed to coincide with the completion of most care plans.\n\nThe purpose of the evaluations was principally to assess whether the intervention was implemented as intended. During one of those visits taking place after care plans creation, the coordinator was asked to set aside an extra half an hour to conduct care plan evaluation. They opened a pre-prepared online form, filled out the list ID (doctor identifier) and the form automatically generated 5 random numbers between 1 and 25 (maximum number of ECM patients and therefore care plans per doctor). After confirming with a doctor which of those care plans actually exist, the coordinator received printed version of the plans and, alone, scored them using 6 Likert style questions pertaining to plans' contents and completeness (see the questionnaire for details), plus an optional 'comments' question at the end.
